What is Form SS-4?
Businesses, trusts, estates and some individuals need an EIN. Many people can apply online at irs.gov, but international applicants and some entity types must fax or mail the SS-4.
Filling it in: quick tips
- Line 1: the legal name of the business or entity.
- Lines 7a–7b: the responsible party and their SSN, ITIN or EIN.
- Line 9a: the entity type; line 10: the reason for applying.
- Sign at the bottom and include a phone and fax number if you'd like the EIN faxed back.
These tips are a quick guide, not tax advice. The form's own instructions on irs.gov are the final word. Form version: Rev. December 2025, checked October 2026.

Frequently asked questions
Can I fax Form SS-4?
Yes. The IRS lists fax numbers in the SS-4 instructions, and can fax your EIN back if you include your fax number. No fax machine? You can fax a PDF from your phone with a fax app.
Is there a fee for an EIN?
No. The IRS never charges for an EIN. Be careful of sites that charge to file the SS-4 for you.
Is my information uploaded?
No. The PDF opens in this browser tab and everything you type stays on your device. Nothing is sent to a server, so your SSN, address and signature never leave your computer or phone.
Is it really free?
Yes. No trial, no subscription, no sign-up and no watermark. You download the finished PDF straight away. (Some “free” PDF fillers charge a small trial fee that turns into a monthly subscription. This one never asks for a card.)
Can I sign it here?
Yes. Press Signature to draw it with your mouse or finger, type your name in a handwriting style, or upload a photo of your signature, then place it on the signature line.
